Method 1: Print your own 2-sided Lineup cards on Letter Size paper (requires 2-sided printing)
- Use card stock for printing if possible
- Download this PDF file and save it to your computer
- Open this file using Adobe Reader.
- Enter your team information.
- Enter your player information. Your players should be listed in jersey order.
- Enter the dates and opponents for your two upcoming matches (or leave these fields blank and fill in prior to your games)
- Print page 1
- Flip your printed card over and print page 2
- Repeat steps 5-7 for as many game cards as you need.
Method 2: Print on AYSO supplied pre-printed forms
AYSO has issued official AYSO Line-up cards that can be printed 4 at a time from your computer. These cards are blank on the front (That is where the line-up info will be printed) and pre-printed with the game report on the back. Here is the process:

- Fill-in the team name, number, coach info, etc
- Enter the players in Numerical order (2, 3, 4, etc)
- For things that change every week (like date, field, time, opposing team), just leave them blank
- Set the paper size to LEGAL
- Do a test print with a Normal piece of paper (Noting which side gets printed on)
- Print the Line-up cards one at a time.
Notes:
- You cannot save the document with all the names in it. So do not close the window or quit the application until you are finished printing.
- Window XP and Vista (all versions) users and Mac OS 8,9 and 10.0-10.4.11 will need Acrobat reader which can be downloaded from
Adobe
- Mac OS X 10.5.x (Leopard) users can use Preview which is built-in
